Transportation forms are available at the school.

• Complete your section of the form and return it to the school.
• The school is responsible for completing the related section (if applicable) and will submit the form to Student Transporation Services.
• Student Transporation Services reviews the form and will respond to the school.
• The school is responsible for following up with you.
• Incomplete forms will not be processed by STS and will be returned to the school.
• NOTE: Is your surname different than your childs?
  Please make sure the correct surnames appear on the form.
